Job Summary

Responsible for the coordination of patient care in a multidisciplinary clinical setting. Works with various providers and services to ensure the best possible treatment and outcomes for patients. Coordinates necessary patient care services with providers, lab, radiology, additional specialty services, pharmacy, infusion centers, etc.

During the first six months, onsite work is required for training purposes. Opportunities for a hybrid telework schedule may be available after the initial training period if supported by operational needs.

Responsibilities

Essential functions
  1. Directs the scheduling of patient appointments with providers and ancillary services.
  2. Reviews and directs incoming referral for metabolic and genetic clinics.
  3. Follows up on patient's treatment, assisting with laboratory and imaging preauthorization and orders.
  4. Triages patient questions and ensures appropriate coordination and follow through.
  5. Gathers needed medical information such as records to assist in the evaluation of patient needs.
  6. Call out imaging and laboratory results to patients when directed by provider.
  7. Coordinates team meetings for metabolic clinic with several providers and specialists.
  8. Coordinate appointments for abnormal Newborn Screen patients with the department of health and primary care provider involvement.
  9. Responsible for data management to maintain and update patient information for metabolic clinic.
  10. Assist in patient care coordination by developing list of local resources, connecting families with each other (when appropriate), connecting families with appropriate resources, etc.

Minimum Qualifications

Bachelor's degree in Health Care Administration, Business, Communication, Nursing or related field, or equivalency (one year of education can be substituted for two years of related work experience). Demonstrated human relation and effective communication skills are also required. Experience in oncology, multidisciplinary clinics, and/or working with high-acuity, complex patients may be preferred.

This position is patient-sensitive and must fulfill all associated requirements.
