Student Health and Counseling Services is seeking applications for a Clinical Case Manager. Under the direction of the Student Health and Counseling Services Clinical Coordinator, this position fosters students' academic and personal success by providing counseling triage, supporting clinical staff, identifying, and responding to students in crisis, and making appropriate referrals. This position facilitates communication within SHCS, across the institution, and between the institution and health care providers. This position works closely with the Wellness Educator to develop and implement health promotion and prevention programs that address campus wellness trends and priorities and to reduce personal, campus, and community wellness risk factors.

Job Functions/Responsibilities
• Counseling triage: assess students for mental, emotional, or substance abuse problems and help students identify the most appropriate source of care and help them navigate the process of accessing it
• Follow up with students missing scheduled appointments in SHCS
• Manage crisis in accordance with assessment needs
• Coordinate referrals for clients/patients with significant health issues or students needing long-term support to local community resources, following through to ensure service efficacy
• Make appropriate referrals to campus resources and services to promote student's academic success
• Document client/patient and treatment interventions using EHR software.
• Serve as coordinator of the CARE Team and Crisis Response Team
• Provide crisis intervention to students experiencing mental and emotional distress
• Provide information and referral support services to students, faculty, and/or staff as needed
• Follow up and coordinate with parents and academic administrators to help students either resume their studies, seek a leave of absence, or withdraw from the institution
• Crisis on-call rotation
• Actively support and contribute to UTM's mission to create an inclusive and supportive campus culture for students of diverse identities
• Engage students, faculty/staff, and community constituents to promote students' wellness and to create campus and community environments that support students' wellness
• Develop, maintain, and promote effective and productive relations with those that have a significant interest in or potential effect on the students served by the programs
• Maintain a cooperative relationship among clinical staff by communicating information and responding to requests
• Collaborate and communicate with the inter-professional team to ensure quality and continuity of care
• Develop, plan, coordinate, and implement a variety of wellness preventative and educational programs for the campus community based on sound theory and evidenced-informed practices of college health promotion
• Advance the wellness of students and contribute to the creation of an institutional and community climate of wellness and social justice
• Use multiple formats, strategies, and contexts to provide universal access to programs
• Participate in professional development activities to enhance professional growth
• Perform other duties as assigned


Qualifications :

Minimum Requirements
Master’s Degree in Social Work, Behavioral Science, Counseling or closely related field
Minimum of two years of experience in a mental health and wellness focused agency or an equivalent combination of education, training, and experience.
Experience assessing and coordinating care for individuals experiencing a mental health crisis

Preferred Qualifications
Licensed or eligible for licensure, as a mental health care provider by the State of Tennessee
Knowledge and relationships with local health care providers
Prior experience in a university setting
