LIFE's Clinic RN team of Case Managers perform a vital service by providing primary care for Tulsa's aging population.
They handle up to 45 patients who meet nursing home level of care. This team's job is to provide services and oversight to keep participants in their homes for as long as safely possible.
Most participants have numerous chronic conditions and have had minimal healthcare in the past. When they come to LIFE PACE, RNs are the ones to get them back on track! A high degree of RN oversight makes that happen.
This involves:
- complete case management and total coordination for the participant;
- responsive call-back to participants who call in frequently; LIFE PACE is all-inclusive program;
- completing 6 month assessments after intake and 10 wellness calls every 2 weeks;
- responding to high E-mail volume from inside and outside the organization;
- performing a variety of treatments to include: wound care (occasionally), catheter changes (infrequently), and lab draws (when MA unavailable);
- successfully adapting one's approach (tone, language, urgency) to respond to diverse personalities;
Who will succeed in the Case manager / Clinic RN role?
Someone who is organized, who can adapt quickly and easily, and who has the energy and mental acuity to respond to the changing needs of the program and the seniors it serves.

On call pay (1 week/ month)
This is a safety-sensitive position and the following are required:
- pre-employment drug test
- proof of flu shot or post-employment vaccine
- Tb skin test annually or proof thereof
COVID vaccination not required, but available.
